Engaging and Sustaining Healthy Communities Using a Baldrige-Based Framework

Despite individual efforts by healthcare organizations to address specific community health challenges, there is no single agency with the depth of resources to tackle the significant and complex challenges our communities face. When operating in silos and treating symptoms, as is often the case, organizations run the risk of limited impact, unnecessary duplication and conflicting efforts. To ensure better outcomes, all community sectors need to have a seat at the planning table, listen to each other and the many perspectives that exist within communities, understand their role in improvement efforts, and work together on shared priorities and strategic objectives to coordinate resources and activities that address root causes and achieve measurable results. Join us in this 90-minute panel to gain insights from top healthcare executives on how healthcare systems can lead us through these challenges toward thriving communities.

Participants will learn effective leadership strategies used by healthcare and public health organizations in Toledo, Ohio; San Diego County, California; and Southern Florida to engage with various community sectors, emphasizing how to maximize the effectiveness of Community Health Needs Assessments (CHNA), value-based care strategies and social determinants of health (SDOH) initiatives. By aligning efforts and leveraging partnerships across a community, healthcare leaders can significantly enhance community health outcomes and advance overarching healthcare goals. In addition to panel presentations, ample time will be allocated to audience questions and discussion. Attendees will discover an innovative approach using the Baldrige-based Community Framework designed to facilitate effective collaboration between healthcare systems, nonprofits and other service providers. This framework will highlight the essential elements for forming successful partnerships and demonstrate the value proposition for all stakeholders involved. Through this collaborative approach, participants will also learn how strategic investments in the community can drive economic benefits to healthcare systems, including improved workforce health, recruitment and retention.
